I have shoulder/forearm pain?

I had a fall in July and have had constant pain from my shoulder to elbow which does not seem to be improving although I am treating it with Ibruprofen Gel and Paracetamol.

You need a thorough exam and MRI to find the cause of the pain. It could be an intervertebral disc prolapse.
If you had a fall and have continued pain you should definitely get an x-ray to see if there is any fracture.
Good morning. You likely have a rotator cuff injury with associated bursitis. Following 4 months of rest, medications and continued pain you should be evaluated by an Orthopedic surgeon and have x-rays taken. Unless evidence of previous fracture they will likely start you on some therapy and then if no improvement consider MRI of the shoulder. Surgery would be last resort if indicated. Good luck.
When you fell 3 months ago did you break the fall with your outstretched hand or did you shoulder actually contact the ground first? Does the pain radiate from the shoulder to the elbow?
Do u you ou have p as in st night if sleeping on your side?
Have you seen a family doctor or an Orthopaedic surgeon?
Have you tried ice packs, or Arnica gel ?
Have u taken an oral anti-inflammatory medicine?
Has anyone taken an ray of your shoulder?

With such limited information it is difficult to determine what is going on. However you may have a posterior shoulder dislocation or even a fracture. X-rays and an Orthopedic consultation are definitely indicated.
I would suggest following up with an orthopedic surgeon since none of those interventions are working. If you haven’t had an x-ray already, they may want to do one, and if not an MRI maybe appropriate. It is possible with that fall that you may have tore a ligament, caused an internal bruise or contusion or you may have a subtle, hairline fracture.
