Based on that sugar you have diabetes. I am not sure if you’ve been told this in the past or not as it looks like the information you have shows you’ve had this for a little while. With numbers that high, you need prescription medications at least for a while as well as likely changes to your diet and lifestyle. If you do not have a primary care physician I would highly recommend getting one. If you have one, you should work with them on getting you on the right regime. Something you can do in the meantime is to make sure you are eating healthy with small portion sizes especially with carbohydrates. Carbohydrates are not evil with diabetes but being very careful about them and the amount is very important. Doing physical activity can also greatly help keep sugars in control. But as I said with sugars that high, you will need medication at least for some time and likely for a while. Please see a doctor soon.
First of all of this new high number, you need to either go to Emergency Room or call your primary doctor as soon as possible. You probably will need to be started on insulin but that would be up to your treating doctor who knows you better. Hope this helps.
