Can be nerve palsy, spinal cause - neck-foraminal, or intraspinal.

The most common background for such a pain syndrome is one or other chronic stress syndrome, most often from work related activities. A complete workup is indicated here including careful history taking, and if it cannot be rolled out offhand a cardiological workup. You should see your GP
