Using anti-fungal cream on a tight foreskin?

I'm 31 and recently went to a walk-in medical facility where a doctor examined me and said I have a mild case of balanitis. He prescribed me Clotrimazole 1% cream to apply to the area 2 times a day for the next two weeks. However something I surprisingly never knew until now is that the foreskin on my uncircumcised penis is supposed to retract over the penis head, but I never did that and trying to is difficult, There's only a small opening when I get fully erect. I know I may have to practice stretching it out over time but for now, would using the cream only the small opening and inner part of my foreskin be advisable? The pain is not very severe or anything, very mild burning at the tip of my head.

You are experiencing what we call phimosis of the penis, which is secondary to chronic fungus infections that you had been experiencing, creating scar tissue and not allowing your foreskin to be retractable. It is best advised that in order to prevent a full-blown obstruction from the foreskin to develop an elective circumcision be rendered and you will not have any more fungal infections.
In rare cases, the foreskin may become too tight and be unable to be retracted. This condition is called phimosis.

Balanoposthitis

This occurs when the glans and the foreskin are inflamed. This inflammation of both foreskin and glans make the foreskin tighter. Although a yeast infection known as candidiasis is often to blame, bacterial or other types of infections can also cause balanoposthitis. Topical creams containing antibiotics or antifungals are often helpful for treating the infection.
I think you need to see a urologist.
You have developed a Ring of scar in the sleeve skin that over lies the head of your penis. This will not allow you to retract the foreskin to clean it properly. You should see a urologist who can define if you will need a dorsal slit or circumcision to rectify this problem
This infection will recur frequently unless you have a circumcision. Are you a Diabetic?
