Well, exposure therapy can be effective at times but it's really dependent upon why the individual is having the anxiety. How is the anxiety presenting? When does the anxiety come out? An evaluation with a psychiatrist is helpful to determine if medication is needed and paired with therapy to understand and gain insight around the anxiety and eventually learn coping skills.
For exposure therapy of any kind, it is typically done in a safe and controlled area (like a therapy office) to start. In addition, for say anxiety, there would ideally be sessions before hand to establish rapport and coping skills that one can use throughout the exposure therapy, like grounding or breathing techniques. With exposure therapy, it is typically done in a "hierarchy" format, simply where you would put the least anxiety provoking situation/event at the bottom, and the most anxiety provoking situation at the highest (like claustrophobia in an small crowded elevator). From here it would slowly be going up that hierarchy with the therapist, in a safe place, using establishing coping skills to help a client through each step of their own created hierarchy. This process can take weeks, months, or even years depending on the the hierarchy, therapy relationship, how often it is being worked on, and intensity of the anxiety, but has proven results to be helpful for many individuals.
