Male or female? Hematuria happens in urinary tract infections, stones, nephritis, prostatic cancer. You should consult a doctor.
Can be from a urine infection or kidney stone.
Definitely. If over age 35 or have any risk factors, cancer must be excluded. You will probably need cystoscope and CT urogram.
