What does this rash (On back of neck) mean?

I've had this rash on the back of my neck for 6 months now and it won't go away. Wondering what is the cause of it? I am not on any meds, have not been vaccinated, only take supplements....b complex, vit C + D, sometimes magnesium, zinc, fish oil and collagen. I have gained a lot of weight over the past 2 years (40lbs) I am 190lbs and 5'4. Could it be a sign of pre-diabetes?

It appears to be acanthosis Nigricans which is genetically linked to type two diabetes. Do blood sugar A1C visit physician about early diabetes.
If this is a dark, velvety rash, then it may be acanthosis nigricans, which is a hallmark of insulin resistance. Check with your doctor to get a hemoglobin-A1c test, which will tell the healthcare provider how high your blood sugars have been on average over the past several months. Acanthuses nigricans can also be associated with obesity, some malignancies, and certain genetic disorders. The condition can also be familial, and even caused by drug reactions. So you will have to seek medical attention to determine the cause of this rash in your case.

There is a rash associated with type 2 diabetes called Acanthosis Nigricans, which can present on the neck. You do not mention family history of diabetes, although you mention weight gain. You should see a dermatologist as well as do blood work for diabetes.
