Hello. As I have aged my posture is not as good as in my younger years. One way (which I tried/do) is to look in a full-length mirror and get the feeling of the posture you want and then have this feeling whenever you walk. The other option is various posture supports. Find one that feels best to you and wear it until you correct your posture. Good luck.
Stretches that target opening the chest and neutralizing neck and upper back muscles daily adding strengthening exercises that focus on muscles off the upper back and chest... it is very important to reconfigure the areas where you spend the bulk of your time to sit and stand upright focusing eyes straight ahead as much as possible. It is of course easier to correct and maintain posture at earlier age. So if this is a long standing issue, over more the than a few years, it is appropriate to have professional assistance. Physical therapy to rebuild posture muscles support. An ergonomist to assist with the set-up of work and living space to optimize your ability to keep good posture through the day... it is an investment in yourself!
