Normally you only need a little local anesthetic right where they are going to put the tube that drains the fluid.
If you have a pleural effusion that needs to be drained, a local anesthetic to the skin in your chest nearest to the effusion will be administered before a needle is inserted to drain the fluid.

To remove fluid from actually around the lung rather than the lungs per se a generally thin needle is utilized. A local anesthetic is injected 1st to numb the skin, underlying soft tissues and the lung lining layer prior to inserting the larger needle to actually remove the fluid.
