What options does he have?

I am a 63 year old male with the following symptoms:
*- Extreme constipation for 5 months {one healthy movement, 4 tiny chicken nugget size movements in that time range} [ history- had a small obstruction on right side: known because he would have to massage the area to allow feces to pass: and it would squeeze tiny- no large pieces of feces ]
- Swelling of feet, and legs
- Leaky leg syndrome
- Sugar issues {assume type 2- not on insulin}
*- Blood in urine= dark urine, orange then brown, now orange {I noticed darkness was blood in urine
- Slight yellowing of the eyes {history: had seen jaundice like 10 years ago- that symptoms for that went away}
*- Now, swollen testicles {very little pain 2/10}

he doesn't want tubes in his nose/mouth,
what are the other & best options of treatment?

Focusing on the GI issue, you need a proper work-up for constipation. It might be simple slow transit of the colon or sometimes there is a defecation disorder on top of that. Testing might be needed to help make a diagnosis, after which treatment can be offered. Another approach is to treat the constipation aggressively with over-the-counter or prescription laxatives, diet change, fiber, etc. Usually daily treatment is needed for severe constipation.

He needs a full history and physical with his primary doctor. Sounds like liver/pancreas/GI tract issues.
