You should worry about any mole when it changes size, shape, color , itches or bleeds or becomes raised when it has been flat. Also if the mole becomes tender, you should also see a dermatologist or your family physician.
A mole is concerning when it is growing rapidly, when it has irregular shape or it has irregular coloration, this is a part of the mole is darker, it also could be concerning if it itches or bleeds or if it is painful.
When it becomes asymmetric
Moles can be a tricky thing. Things to consider are size, shape, and color:

-Asymmetry: Is the mole consistent in it look or does one side look different from the other? Asymmetry is not good.
-Size: Is it bigger than 6mm, or the size of a pencil eraser? If yes, that too is not good.
-Color: Color that is not uniform is not good either.
-Bleeding: Bleeding is never good.

Does it hurt to touch? If you are concerned, follow-up with a dermatologist is recommended.
