Hi,

It could just be that you’re straining a muscle when you play, but it might be indicative of a small disc bulge in your back. I would need to get more details to give you a better opinion.
At this age, often times there is stress injuries to the facet and vertebral bodies due to the amount of torque that it requires to repetitively hit a tennis ball. It would be best to consider getting imaging, including x-rays, and possible MRI, so that you can work with a good physical therapist to prevent not only progression but help with pain. Pain at 20 years old is not normal, don’t accept it. The longer you wait to treat it, the more severe it will get
Torn muscles or disc herniation or disc bulging.
There are many possible reasons. Without a thorough evaluation, some of the reasons could be: asymmetric forces on your back, poor muscle flexibility, weakness, vertebrae rotation, or disc herniation. You should get a formal evaluation.
There may be a structural defect such as a Spondylolysis. X-rays and MRIs are very effective in evaluation this problem.
You haven't given me nearly enough information to be able to make a guess here. What part of your back? What is the nature of the pain -- burning, aching, throbbing? How severe is it? How long does it last? Does Tylenol or heat make it better? Does the pain radiate anywhere or stay in its one spot? Are there any other associated symptoms? How long has this problem
been going on? Are there any particular movements that make it better or worse? Take the answers to all of these questions to your doctor who can do a physical exam and then possibly determine what's going on.
