The best home care is to floss first, brush second, and rinse with something like Listerine, making sure the rinse gets between all of your teeth.

If you start with your teeth professionally cleaned, you can maintain the tartar-free teeth with daily floss (ideally, or some other interdental cleaner) and careful brushing twice a day. You don't. This is a procedure only a professional can do for you. It involves anesthetic and deep scraping with sharp instruments.
Unfortunately, you really need to visit a dentist, and either the dentist or hygienist will have to do the deep cleaning
