Knee pain?

I have a 14 year old daughter who runs cross country. She hurt her knee about two months ago. She has been in physical therapy for four weeks. Hasn't run on it since April. She tried to run on it yesterday and she had a sharp pain in the front of the knee when she has no pain? Why does she still have pain?

If she is still having pain, it would be best for her to see an orthopedic provider for an evaluation and treatment plan. A physical exam and, if needed, an MRI, will be needed for a proper diagnosis.
She needs to see an orthopedic surgeon.
Cross country runners usually get overuse type injuries. In a 14-year-old, Osgood-Schlatter's is very likely which is due to inflammation at the tibial tubercle as the bone grows faster than the patellar tendon. An explanation can be found on orthoinfo.org. Ice and stretching are the main tenets of treatment. Osgood-Schlatter's is self limited, so it stops when the rapid bone growth slows down. A return visit to an orthopedic surgeon can confirm the diagnosis and provide further insight.

Multiple options exist is to the ideology of her pain. It would appear she need some more thorough valuation to include an MRI scan of her knee.
A common problem in female runners in this age group would include the Possibility of a stress fracture. Hormonal changes missed menstrual periods and often nutritional deficits (Poor calcium and protein intake )may be factors in teenage runners.
An MRI scan would rule in or out the possibility of a stress fracture and determine if there are other possible causes from structural injury. Getting the scan would be the best move at this point.
It may be something structural like an osteochondral defect or patella chondromalacia.
It depends on original injury. I would recommend having an orthopedic doctor look at her.
