We don't really know, but they probably form as a result of high pressures in the rectum associated with a lot of straining with bowel movements, especially in environments with low fiber diets. For symptomatic hemorrhoids, you can see a GI specialist and get them banded serially - simple procedures.
It mostly occurs with a history of chronic straining during bowel movements, and from pregnancy; sometimes it can be familial, and may occur in multiple members of the same family.
Often hemorrhoids run in families. The best course of action is a high fiber diet. You may benefit from an examination by a colon and rectal surgeon. Depending on the size of your hemorrhoids, there may be some office procedures that will improve your symptoms.
It's either from the diet causing bowels not to be 'formed, soft stools' OR behavioral habits of straining too much.
There are many issues in the anal region that are mistaken for hemorrhoidal disease. Hemorrhoids are normal parts of our body but can become irritated with chronic straining and constipation. Weight gain has also been thought to be associated with hemorrhoids.
