  • Is it worth getting a rhinoplasty?

    It depends on the severity of your functional issues with breathing as well as any cosmetic concerns, but assuming that this issues and concerns are valid and reasonable ones, then, generally, yes! Seek a Board Certified Plastic Surgeon to conduct a thorough evaluation and nasal analysis to determine what rhinoplasty plan is best for you. There are many approaches and techniques that we use to reshape the nose, particularly the underlying nasal skeleton, which is composed of both bone and cartilage. For a small subset of patients with more subtle irregularities to the nasal contour, injecting dermal filler can be an option rather than surgery, but one should exercise caution with this approach as injections to the nose do bear risks of skin necrosis (death of some of the skin) and rarely, even blindness. Again, whether pursuing a surgical or “liquid” (non-surgical) rhinoplasty, select a specialist with proper credentials, such as a Board Certified Plastic Surgeon who understands facial anatomy extremely well.   • Which treatment is best for skin whitening?

    By your question, I am guessing you mean skin "lightening" versus "whitening." The go-to medical treatment to the skin to help correct uneven skin tone, which often lightens or brightens hyperpigmented (darker) skin, is use of topical hydroquinone or 'HDQ.' The common formulation is 4% hydroquinone and must be obtained by prescription or by a physician, such as a Dermatologist or Plastic Surgeon, who carries skin care products in his or her practice. Hydroquinone (HDQ) does essentially bleach the skin, but it does it in a controlled manner, as the medication targets the melanocytes in your skin that produce pigment, and it stimulates more even production of melanin throughout the skin of the treatment area. You have to be patient with results, as you may not notice a visible change or improvement in skin tone unevenness for at least four (4) weeks, sometimes longer. Hydroquinone can also be a useful "pretreatment" to the skin before individuals with darker skin type (such as some Hispanics, Indians, and Blacks) undergo laser resurfacing, as laser treatment can worsen dark pigmentation. Determining your skin type is a very important first step, and we Plastic Surgeons do this by figuring out your 'Fitzpatrick' score (1 corresponds to a redhead who burns easily in the sun and never tans, while a 6 is the dark Indian to Black skin type that never burns and always tans).   • What is the best nose non-surgical procedure?

    There are a variety of non-surgical treatments to improve your nose's appearance, from its external skin to its overall shape and contour. Starting with improving skin quality and texture, options include skin resurfacing, whether using laser, radiofrequency, or dermabrasion. These options usually require multiple treatments, spaced at least 4-6 weeks apart, for best results. Another option is to apply chemical peels, which resurfaces skin chemically rather than mechanically. Again, as with the previously mentioned treatments, peels also require multiple sessions. Think of your skin like a canvas, and everything underneath providing structure. While surgery can address both, the result can be underwhelming if the canvas is not smooth and healthy. Non-surgical interventions can help make the canvas appear as smooth and healthy as possible, which by itself can make your nose appear noticeably improved, and at the least would make a surgical result such as a rhinoplasty (nose job) appear even better. What you may see a lot of on social media is improving the nose by injections of fillers. Dermal fillers are usually composed of hyaluronic acid (HA), but there are alternatives out there that are more semi-permanent, such as Radiesse (a calcium-based product). HA fillers can give you immediate, noticeable results right after injection; however, HA fillers also wear off, anywhere from 9 months to 2 years post-treatment.   • Do cheek fillers lift the face?

    Hyaluronic acid (HA) fillers are an excellent way for a young woman such as yourself to achieve a simulated "lift" of the cheeks. In fact, this is why some of the HA fillers on the market, such as Restylane "Lyft", market themselves as helping patients achieve a lift. However, HA fillers are no substitute for a surgical facelift, which truly elevates the soft tissues of the face to address descent of the cheek or "malar mound." For a 29-year-old, though, a facelift is generally not recommended for someone that young. A facelift addresses descent of the malar mound with associated loss of soft tissue volume over the cheek, while smoothening facial wrinkles or "rhytides." The lifting effect of fillers, by contrast, is caused by proper injection of the filler deep into the cheek onto the "hard" structures of the face, i.e., the cheekbone, "zygoma." The injections are typically done with a series of small boluses or "aliquots" spread across the contour of the cheek (again, deep onto the bone), and this injection pattern of HA is what achieves the simulated lift. HA fillers are not permanent, however, but they can last for up to two (2) years with deep injection and use of a larger particle HA such as Restylane Lyft or Juvederm "Voluma." Fillers to other areas of the face can wear off within nine (9) months, particularly in more mobile areas of the face such as the lips. As the names of these brands suggest, HA fillers to the cheek can not only simulate a lift but they also inherently add volume to the cheek.
