Unexplained vomiting during bowel movement

I am writing for my mother who cannot speak English, so I will do my best to translate what she explained.

Sometimes, when she has a bowel movement the same thing happens. She starts having abdomibal pain that goes all the way up to her stomach and makes her throw up (even when she doesn't, she feels like she's going to). She also says her head feels like cotton, her nose starts to drip and she is hot. It happens during the bowel movement, not after. When everything is gone she feels okay.

She cannot find any trigger, and it's completely random. It just happens sometimes. She has asked a doctor who said he doesn't know what it could be.

We looked into vasovagal reflex but she has those others symptoms (nose drip, pain) that don't add up. She would like to know for she is worried and embarassed.

It's important to know if she is straining a lot to get the stool out and if these symptoms happen when she is straining. If so, then it is probably a vaso-vagal response, including the pain and nasal drip. Is she constipated? She may have a defecation disorder too, so that would need testing such as a defecogram and anorectal manometry.
