Extended heavy menstrual cycle

My wife has had a heavy flow period for almost 3 weeks now. She is 48 and we have 6 children. 4 were natural births and 2 were c section. She was diagnosed and treated for uterine cancer 5 years ago, and was declared cancer free. After the last childbirth, she had her tubes tied. What is going on?

Thanks for advocating for your wife. She needs to see a gynecologist. If she was treated for uterine cancer and has her uterus, one must make sure there is no recurrence.
Post tubal ligation may be the source of the heavy bleeding or maybe from another source like fibroids. With your history you could watch closely or have the uterus removed and end your 2 worries
I advise her to visit her ob/gyn. Sometimes it can be hormonal changes for menopausal reasons or there can be underlying pathology. She may need imaging, too, but I don’t think it is malignancy again.
I would wish to have more information about he uterine cancer your wife had. It appears she maintained her uterus after the diagnosis and treatment and was able to have children thereafter? Great There may be various causes to her heavy and prolonged period. A thorough work-up is required which should include a detailed history and physical exam, Ultra-sonogram of the abdomen and pelvis, and an endometrial sampling; and given your wife's history a detail dilation and curettage hysteroscopy will be more appropriate with or without the myosure or other hysteroscopic sampling device.
The management will depend on the findings from the workup and both her and her physician may decide the best option.
Did she have a hysterectomy done for uterine cancer? This bleeding is abnormal, she needs to see a gynecologist ASAP to rule out recurrence.
She should have an endometrial biopsy and an ultrasound of pelvis organs and reevaluate.
She needs an appt soon as she can. She needs an exam and possible biopsy
She needs to see her gynecologist as soon as possible.
