When it comes to breast implants, there are so many questions that need to be answered before having the procedure done. While it is extremely important to discuss any questions you may have with your doctor, let's start with the basics.
What are breast implants?
Breast implants are artificial devices surgically inserted into your breasts. They are silicone shells filled with either silicone gel or saline.
Who gets breast implants?
A person who has lost a breast due to cancer may wish to receive a breast implant. Someone who is healthy may wish to change the size or shape of their breasts.
Who does the surgery for a breast implant?
A plastic surgeon does the procedure for a breast implant. The surgery is called breast reconstruction for a breast cancer patient. It is referred to as breast augmentation for healthy individuals seeking a change in breast size or shape.
Who is a good candidate for breast implants?
You may be a candidate for breast implants if:
- Your general health is good.
- You have realistic expectations.
- Your breasts are fully developed.
- You are bothered by the feeling that your breasts are too small.
- You are dissatisfied with your breasts losing shape and volume after pregnancy, weight loss, or aging.
- Your breasts are asymmetrical.
- One or both breasts failed to develop normally.
- You have had to have a breast or both breasts removed due to cancer.
What types of breast implants are available?
There are two types of FDA-approved breast implants. They are saline-filled and silicone gel-filled. Both of them have a silicone outer shell and vary in size, thickness, texture, and shape.
Filler: Saline vs. Silicone
Saline implants are filled with sterile saltwater. They are inserted empty and then filled once in place. This type of implant is available to individuals age 18 and older for breast augmentation and to any age for breast reconstruction. Silicone implants are pre-filled with silicone gel. The gel is a thick, sticky fluid that mimics the feel of human fat. This type of implant is available to individuals age 22 and older for breast augmentation and to any age for breast reconstruction.
Surface: Textured vs. Smooth
Textured implants have small bumps on their surface. They were developed to help reduce the risk of contracture after augmentation. Smooth implants have a smooth surface. They provide a softer, more natural feel.
Shape: Round vs. Teardrop
Round implants conform best to the natural shape of the breast. They usually offer great results. Teardrop implants may be a better fit for individuals who have had parts of the breast removed during a previous surgery, such as mastectomy reconstruction.

Breast Implant Placement
There are two primary placements for implants. One is over the chest muscle, known as subglandular placement. The other one is under the chest muscle, known as submuscular placement.
Subglandular Placement
When an implant is placed in a subglandular position, it is located between the breast tissue and pectoral muscle. Benefits of this placement include a more straightforward procedure to perform, a more comfortable recovery, larger implants may be used, implants can be placed closer together for cleavage, and it does not create external distortion when the chest muscle flexes.
Submuscular Placement
Submuscular placement refers to implants that are put underneath the chest muscle. Some benefits of this placement include more coverage over the implant to create a more natural appearance, the skin is less likely to stretch or sag, lower risk of capsular contracture, can not see the edges of the implant, and less chance of visible rippling forming.
