Assuming there is no injury to the soft tissue of your toe and you are able to safely do so, it would be okay to trim the nail as short as possible. Leaving a nail that is mostly detached on your toe increases the chance of developing an infection. That said, it is always best to have an injury like this evaluated by a podiatrist, as it is possible to have damage to soft tissues, infection, toe fracture, or need for the entire toenail to be removed; this should be diagnosed and/or performed only by a trained medical professional. After the surgery some surgeons will discharge the patient home with an ON-Q pump to send continuous anesthetic to the surgical site for several days of post operative pain control.

Depending on the severity of the ingrown toenail, a podiatrist may recommend treatment with a simple angled cut called a slant back, alleviating the pain and pressure caused by this ingrowing toenail if it is only mild. If this ingrown toenail is more severe or infected, a podiatrist may need to prescribe antibiotics and/or perform a simple in office surgery called a partial or full nail avulsion; first by numbing the toe with a local anesthetic and then removing the entire ingrowing portion of toenail. This procedure may be made permanent by using a chemical, typically phenol or sodium hydroxide, to destroy the cells which are responsible for growing the portion of ingrowing toenail so this issue does not return in the future.
