Pain in my back on the right side?

I lifted a light box and instantly felt a sharp pain. Now it's a stabbing sharp pain kinda like a spasm. It tightens and then lets go for a minute. What could it be?

Muscle strain with back spasm, or could be a degenerative disc with nerve impingement. Hard to say without an examination.
If you have had prior symptoms, it is likely an aggravation of the prior problem. Arthritis, muscle strain, disc protrusion.
Sounds like you strained your back. Take the first usual measures. Apply ice packs for the first week and then moist heat. Take Advil but no more than 6 per day with food and/or Tylenol no more than 4 per day. Do some stretching and walk to tolerance. If the pain does not resolve or worsens seek medical evaluation.
Likely muscle strain.
