By a careful exam which can be done by Zoom
With an MRI. You can have one prescribed after an office visit
MRI is most accurate imaging.

Night time pain when you sleep on it and pain with raising your arm above your head. Then physical exam and MRI
A practitioner will diagnose a rotator cuff tear with a good physical exam first. If there's a high suspicion for a tear, and MRI of the shoulder is the next diagnostic step. That said, it would be very uncommon for a 28-year-old to have a rotator cuff tear. More common pathology would be a labral tear orAC joint related pain
History of the symptoms and physical examination followed by an MRI or ultrasound of the shoulder.

Best way is MRI. Can get some idea with an exam.
