Blood work and you will usually have fever
You may develop pyelnrphritis, with fevers, nausea, vomiting, and lower back pain on both sides, and tenderness along with burning urine.
Most of the times you start having fever and pain in both flanks.
One symptom of spread to the kidneys would be new onset of flank/back pain. You can also experience fevers/chills, blood in urine, abdominal pain, and other symptoms.
