I am a 55 year old male with heart rhythm problems. When do you know you need a new heart?

There are many types of heart rhythm problems. If your heart muscle is very weak that causing significant rhythm changes despite optimal treatment and you are unable to carry out your daily activities without running out of breath then a heart transplant or artificial heart is indicated.
When medications or surgery cannot fix heart failure.
Heart transplant is indicated for patient with terminal heart disease (severe cardiomyopathy)
I would suggest you see a good cardiologist for an evaluation and see how that works out. A new heart should be the last thing you want at your age unless your heart is badly failing with no other options.
