Dr. Nicolas Lailas MD's Expert Contributions
  • Stints in bladder after kidney stone surgery?

Your stents should not be in this long for kidney stones. You need to go have the stones addressed or have the stents exchanged. Most likely they are infected or obstructed. Dr. Niko Lailas READ MORE

  • I have a hard lump on my left side next to my scrotum?

It looks like you shave your groin. You most likely have an ingrown hair follicle. Unfortunately, you probably are going to need antibiotics and drainage, especially since you are a diabetic. Dr. Niko Lailas READ MORE

  • Is this HPV?

No. HPV looks like mini cauliflower. Dr. Niko Lailas READ MORE

  • It burns to pee?

It is not, you need a urine culture. Dr. Niko Lailas READ MORE

  • Should I see a dr?

Yes, you may have disrupted your urethra. You will need a retrograde urethra gram, which is a study to see if there is a hole in the urethra. If there is, it is a simple fix by placing a catheter and allowing the urethra to heal for 7 to 10 days. Dr. Niko Lailas READ MORE

  • Chlamydia concern?

Sure, you need to be evaluated with a culture. Dr. Niko Lailas READ MORE

  • I have loss of sensation in penile and testes?

This typically happens to bike riders from prolonged sitting on the bike seat. I think you need a pelvic MRI to fully evaluate. The hour glass look of the penis is due to the fibrosis. I suggest a vacuum device and straighter to help with this. If that doesn’t help, a duplex Penile Doppler may be of help. The erection is totally separate from the sensation. Dr. Niko Lailas READ MORE

  • Enlarged prostate?

You can try Flomax 0.4mg a day and if that does not work, you need a look in the bladder to rule out a urethral structure causing the obstruction. Dr. Niko Lailas READ MORE

  • Pain while urinating and during sex?

To rule out cervical cancer, you need a Pap smear and at the same time you can get a urine culture. That will address those issues. Dr. Niko Lailas READ MORE

  • Enlarged left testicle?

Sounds like it could be a hernia. My suggestion is to get a scrotal ultrasound. Dr. Niko Lailas READ MORE

  • I no longer have morning wood?

Make sure you are exercising regularly and get a good amount of sleep. If those are in order and that is not an issue, then you may consider getting a morning testosterone level. Dr. Niko Lailas READ MORE

  • I have blood in my urine?

So, a few things: Six years ago was six years ago. You need to repeat all of the above testing. So, you need a cystoscopy, CT urogram, and a urine culture and cytology. Personally, I think you may have passed a small stone and that would explain the back pain, the urge to urinate uncontrollably, and the blood. Unfortunately, that is a guess and therefore, you should repeat your work-up. Dr. Niko Lailas READ MORE

  • Do I have an STD?

What you have is folliculitis from shaving down there. This is very common. Treatment is typically warm compresses and antibiotics or excision if the antibiotics don’t work. This has nothing to do with a low sex drive. Dr. Niko Lailas READ MORE

  • My husband has bladder cancer is there a bladder replacement surgery?

It all depends on the stage and grade of the tumor. If he has superficial bladder cancer, this accounts for 80% and only needs removal and scraping without removal, but the bladder muscle invasive tumor requires chemotherapy followed by surgical removal. At that point, the substitutions available for bladder replacement are an ileal loop or a neobladder or a possible catheterizible stoma. Dr. Niko Lailas READ MORE

  • Could I have gotten an STD?

Unfortunately, people lie all the time, so you should never take their word for it. As long as you don’t have any urinary symptoms or pain or discharge between five and seven days afterwards, you’re fine. Dr. Niko Lailas READ MORE

  • Infections?

It is impossible to transmit a viral infection through a yeast infection. You can transmit a yeast infection to a male, which manifests by a red, flaky foreskin. Dr. Niko Lailas READ MORE

  • I have a question about the scrotum u/s doppler?

That usually is consistent with a varicocele, which is a dilated been typically on the left side. The word cystic does not go along with a varicocele so it’s not completely 100% clear. Dr. Niko Lailas READ MORE

  • I have a pimple in my testicles?

In your situation, if you are shaving your “junk,” this is most likely causing ingrown hairs that can become infected. You need to get antibiotics and warm soaks. Dr. Niko Lailas READ MORE

  • How do I know if I have an STD?

Absolutely not. You’re okay. Dr. Niko Lailas READ MORE

  • Can I continue self catheterize indefinitely?

You can and some people do. My advice though would be consider doing a minimally invasive procedure to not have to catheterize. Those procedures would be a urolift, bipolar TURP, green light laser turp, rezume or a prostate embolization. Dr. Niko Lailas READ MORE
