It sounds like you might have bruising on your arm, which could be from injury or pressure on the skin. However, since you also mentioned heart problems and diabetes, it's important to get it checked by a doctor. These conditions can sometimes affect how your body heals and could make bruises appear more easily or heal more slowly.

Here’s what you can do in the meantime:

Rest the arm and avoid putting pressure on it.
Apply a cold compress to the area to reduce swelling and pain.
Monitor the spots: If the bruises don’t get better, get larger, or cause more pain, seek medical advice.
It’s always good to consult your doctor, especially with a history of heart problems and diabetes, for proper care.

A dermatologist appointment will be required.
Black and blue spots are usually due to some sort of injury. If the pain and spots persist, definitely visit a local doctor for evaluation.

If the spot itself is tender to touch, it could be an infection or a vascular inflammation. I recommend a visit to your doctor or to a dermatologist ASAP to check it out.
