Allergies or hives?

My son has rash all on his arm and he always scratching it. What does it mean?

Those are very typical of hives and he needs to find an allergist to test him for any underlying cause.
The picture is a little hard to clearly identify the rash. It looks like eczema. An allergist or a dermatologist would clarify the diagnosis. Itching is horrible. Don’t waste any time getting him treated
The reasons for an itchy rash in a 6-year-old boy may be quite diverse. From an allergic reaction to food, drugs, or environmental allergens to insect bites or immunolologic condition. In order to make a correct diagnosis, your child should be seen by a specialist and have a comprehensive evaluation and testing.
It could be several different things. It depends on how long he’s had it, does it come and go, does it itch, how long do the bumps last, does it blister, bleed or scale, is he an allergic child in general, is there a family history of allergy, eczema, hives, poison ivy, drug allergy... There is much more to it than what information is available. Seeing and allergist or dermatologist or just his pediatrician could be invaluable.
Please see your local primary provider to discuss whether or not you need to be seen by or referred to a specialist/sub-specialist. Surprisingly, skin conditions that appear to be allergies are actually rarely allergies. This may or may not be your case. A dermatologist might be your best option, or may not be. Your primary provider can help you determine this.

The appearance of the rash is consistent with hives. However, your son will benefit from an evaluation by either his pediatrician or an allergist. Full history and physical exam helps to evaluate possible triggers and also can alleviate fears and assess risk. I hope he feels better soon.
