Why do you think halitosis is from chronic sinusitis? Have you been diagnosed by an ENT with chronic sinusitis, which usually requires a CT sinus in order to see evidence of chronic disease remaining after maximal medical therapy (consisting of 3 weeks of antibiotics and oral steroids and topical nasal steroids)?
Halitosis is much more commonly from untreated reflux disease, possible H Pylori infection, or tonsil stones. Much less likely is your bad breath from sinus infections. Get a CT sinus ordered by your primary care and go see an ENT physician

Use nasal steroid spray daily for minimum 14 days, and you may also try using Afrin nasal spray twice daily for 3 days only, while trying to equalize the pressure in your ears. This will help to resorb the fluid remaining in the middle ear space.

From the American Academy of Family Practice.org:
The signs and symptoms of acute bacterial rhinosinusitis and prolonged viral upper respiratory infection are similar, which makes accurate clinical diagnosis difficult. Because two thirds of patients with acute bacterial rhinosinusitis improve without antibiotic treatment and most patients with viral upper respiratory infection improve within seven days, antibiotic therapy should be reserved for use in patients who have had symptoms for more than seven days and meet clinical criteria.
Four signs and symptoms are the most helpful in predicting acute bacterial rhinosinusitis: purulent nasal discharge, maxillary tooth or facial pain (especially unilateral), unilateral maxillary sinus tenderness, and worsening symptoms after initial improvement.

The best way to investigate this problem is have an ENT examine the internal anatomy of your nose. I doubt the physical act of talking is having anything to do with your congestion. What does impact the anatomy of the nose is blood pressure and humidity. inside the nose are turbinates, which will fluctuate in size and volume based on the body's position, blood pressure and humidity in the air you are breathing. These can change the nasal airflow significantly. Use a nasal steroid spray daily for at least 4 weeks before you go to see an ENT to examine the anatomy of your nasal passages.

Equalizing the ear pressure has nothing to do with your ear wax, as this is a product of middle ear pressure. Middle ear pressure is regulated by the eustachian tube. Ear wax is produced differently by different people and should not be "washed out" but should be removed under a microscope by an ENT physician or audiologist. If you have hearing loss, it is likely NOT from your ear wax but from unregulated negative ear pressure (eustachian tube dysfunction) or sensorineural hearing loss.
Get a hearing test (audiogram) before you start blaming your hearing loss on ear wax. There could be much more going on that could be addressed.

I would go see an ent who can examine your ear thoroughly. Tympanomastoidectomy can change the way an ear drum appears, which may be hard to see what's normal or abnormal by a primary care doctor. It depends what kind of ear surgeries you had. If you had what's called a canal wall down mastoidectomy, you will have a lot of empty space in there which can collect fluid. Either way, i would have an ENT take a look

Well, it sounds like you are having a lot of various symptoms which could be from numerous components, such as anxiety, depression, high blood pressure, low blood pressure, rare endocrine problems, or migraine syndromes. But depending on your body fat and anatomy, having the blood vessels visible in your neck is not that abnormal. A CT angiogram is a type of imaging that looks at the shape of the blood vessels. If you really want to see if you have an aneurysm, ask your doctor to order you one.
