It is common sometimes after an intracranial bleed to have some residual blood that is left over. Sometimes it's more dangerous trying to get it all out. With time it should reabsorb and a small portion may calcify which is also a common happening. Keeping your mom in the ICU for a bit longer is a wise precaution to make sure there's no leakage of blood and that she'll be fine. Good luck to you and your mom. Dr. F. Sent from AOL on Android

There are quite a few questions I think of with your diagnosis and presentation. How was the diagnosis of psychogenic nonepileptic seizures reached. Did you undergo Long term EEG video monitoring and one or several of your events captured demonstrating no seizure activity? Also have you been evaluated for vasovagal or neurocardiogenic syncope?. That can often be misinterpreted as PNES ( psychogenic non epileptic seizures). Looking at the Medications you are taking you are being treated for seizures / mood with Lamictal and Topiramate. Depression and anxiety with Effexor and Hydroxyzine along with CBT. Interactions ( propanolol ) can be used for anxiety, headaches, Tachycardia such as in POTS ( Postural Orthostatic Tachycardia syndrome) or Vasovagal/ Neurocardiogenic syncope. Dexilant us for reflux heartburn. I suggest you have a visit with your treating physician(s) and discuss in detail your presentation and test results and reevaluate your diagnosis and treatment plans. Good luck. Dr F. I don't think you have a tumor. It seems that the Keppra is not controlling your seizures. You might want to discuss with your provider trying a different medication. Valproic acid ( Depakote) usually works quite well; provided you don't have a specific contraindication. There are also other medications that can be considered. On the more holistic side CBD oil or its pharmaceutical equivalent ( Epidiolex) might be quite helpful. If your provider is not willing to discuss and or consider such; Then a new provider might be an option. Good luck. Dr. F. Sent from AOL on Android

Yes, Botox has been helpful in alleviating spasticity and particularly on the heelcords. It must be accompanied by physical therapy and sometimes with serial casting. The draw back sometimes is that after several rounds it looses efficacy. Another option ( a bit more invasive) but quite useful ; Specially if your spasticity is disseminated and involves trunk and upper extremities is an intrathecal Baclofen pump.Talk to your Neurologist or Physical Rehabilitation Dr. about those options. Good luck. Dr. F. Sent from AOL on Android

From what I can see, the cerebellar tonsils are right at the borderline of the foramen Magnum. Thus not meeting criteria for Arnold Chiari I. I'm not sure that getting an upright MRI will change anything ( unless he is symptomatic ) and even then you ( he ) would have to have more than 10 mm downward displacement or evidence of a syringomyelia before any surgical intervention consideration is given.
